Paperwork Required
Travel Papers
(Teams travelling from outside of Oregon only.)
The first thing that you must do is download a copy of our tournament's USYSA "Application to Host a Tournament of Games" by clicking here. This form is required by your state association in order to process the "travel papers" which you must apply for with your state association in order to obtain permission to play outside of your home state. You should submit a travel papers request to your state association immediately. Your state assocation will require that you provide a roster of players actually travelling to the tournament.
Your approved travel papers must be received by the tournament before June 10, 1999. All non-oregon teams must get travel papers in order to participate in the THUSC Invitational.
Medical Release Forms
At all times during your trip, you must carry medical release forms in the event of an injury to one of your players. There are many different forms available but we recommend the USYSA form, or one similar, which includes space for a detailed medical history, along with health insurance information and date of the player's last tetanus shot.
Age Verification
All teams will be required to prove the age of each player before participating in the tournament. Acceptable forms of identification are limited to:
- Birth Certificate (photocopy acceptable)
- Passport
- USYSA ID Card
There are no other acceptable forms of identification. Prior to the tournament, we will send you an official tournament roster form that you will need to submit to us before the tournament. More details on this procedure will be provided to you in late May.
